The breach was identified in October 2025 and involved a data center operated by Unlimited. The investigation revealed that unauthorized access occurred between October 5 and October 10, 2025. During this period, cybercriminals extracted sensitive data from the company’s systems.
The compromised data includes a range of personal details such as names, addresses, and contact information, alongside medical and health insurance data. Specifics such as Social Security numbers, medical record identifiers, diagnosis details, service dates, insurance policy numbers, claims information, and scanned documents like driver’s licenses were also accessed.

To date, there have been no reports of misuse of the compromised information.
